A near miss is a valuable thing to reflect on, and the fact it was caught matters. Before we go further: make sure nothing here identifies the patient, staff, ward or date.
The aim is learning, not blame — yours or anyone's. Try this: describe the point at which the error was caught, and what allowed it to be caught. Then we'll look at what the event shows about the checks in place and your own part in them, written as learning rather than fault.
