Several details here could identify someone, even in one sentence: the exact date, the ward and hospital name, the patient's initial and age, and a specific condition. Together those could point to one person, so removing the name alone would not be enough.
Try this: “I cared for an older man with a life-limiting neurological condition.” Keep the date and place out entirely. Rewrite your opening that way and paste it back, and I'll check the rest of the passage.
The tutor questions, diagnoses and teaches, but never writes your experience, feelings, insight or learning. That boundary is the whole point.
