The meaning is fine, but the voice is doing the opposite of what reflection needs. “One enters… it is then realised” removes you from your own account, and reflection is supposed to be first-person.
Try this: rewrite those sentences in the first person and past tense — “I went onto the ward and noticed…” — keeping it honest rather than formal. Paste your version and I'll look at the tense consistency next.
